The Foundation accepts Letters of Intent to Apply throughout the year. Please complete the following form to be reviewed by the Foundation.

Upon review and if invited to submit a full application, the organization will then receive access to complete the application.

Please attach a Board Authorization letter signed by the Board Chair – a statement from the organization’s board of directors authorizing the request and agreeing to implement the project if funded.
